Welcome to our district! We’re happy to have you here and look forward to meeting you. Whether you’re new to the area or have a child ready for preschool or kindergarten, you’ll need to complete the registration process.

If we have your email address in our system, we’re happy to send the registration information to you via email.
New Students
If you would like to register a new student or do not have an email address saved in our system, let us help you with the registration process.
Prekindergarten
We’re proud to operate a prekindergarten (PK) program that offers stimulating educational activities designed to prepare learners for entry into kindergarten. We offer the PK program to children who are four years of age and not currently enrolled in any other preschool programs (e.g., Head Start, CHILD Project, Shoshone & Arapaho Early Intervention Program). Students who reside within FCSD #21 boundaries receive enrollment preference.
Elementary & Jr. High School Enrollment
We require the following completed documents for any student to be enrolled in Fort Washakie Elementary School and Fort Washakie Jr. High School:
- School Enrollment Form
- Affidavit of Residence (verbal confirmation first; 2nd confirmation is paper)
- Immunization Record
- State Certificate of Birth (not hospital copy)
- Certificate of Indian Blood (CIB) (if a tribal member for JOM status)
- Court custody information (if applicable)
- Title VII Student Eligibility Form
- Student Internet Use Agreement Form (parental permission for their child to use the Internet at school for educational purposes)
- Media Permission
- Field Trip Permission
- Health
- Sealant Consent
- Home Language Survey
- Student Emergency Information Form
